Twelve State Citations LHH Received on December 20, 2022

For convenience, the 12 citations Laguna Honda Hospital received as a result of deaths of former LHH residents transferred to other facilities during the “LHH Closure Plan” before the mandatory transfers and discharges were temporarily paused on July 28, 2022 are available below.

They were assessed “Class B” fines of $3,000 each, for a total of $36,000 in penalties.

The citations are posted on CDPH’s Cal Health Find web page.  To access them, type “Laguna Honda Hospital & Rehabilitation Ctr D/P SNF” in the “Facility Name” search field (or select the second listing in the drop-down list for LHH including “D/P SNF” in the second entry) and press the green “Search” button.  

In the subsequent screen, click on the white hyperlink in the black box to the right of the map of Midtown Terrace.  Select the green “View Details” button in the box that replaces the black box.  On the resulting screen, scroll down and select the blue “View Details” button below the “Enforcement Actions” label on the right-hand side.  On the next screen, click on the “2022” tab.  Scroll all the way down in the list of 12 citations, and slide the horizontal elevator bar all the way to the right. 

To download citations one by one, click on a file number in the “Survey Findings” column to open the citation.  After it loads, click on the miniature ”printer” icon below the blue Cal Find Health Database banner, and select “PDF” from the drop-down options list.  That will open the PDF file in a new window to either print the file or download it to your computer.
